The ability to add Diagnosis Codes in your claim forms in the fields 34 and 34a and sending electronically can be achieved by clicking “REFRESH PATIENT INFO” before sending the claim

Available on the 2019 ADA Claim Form in version 12.371a or higher

2. Create claim
3. Add Code to box 34, then 34a
4. 

5. Click "Refresh Patient Info"

6. Submit Claim
